What this assessment is

Opioid dependence can develop even when the medication is taken exactly as prescribed, which is why these questions ask about the pattern of use rather than only the amount.

It is a short, confidential set of questions and it takes a few minutes: whether you have needed more for the same effect, whether you have taken more than you were prescribed, whether you have tried to cut back, and how it is affecting daily life, relationships and health. It covers prescription painkillers and heroin alike.

This is a starting point for reflection, not a medical diagnosis. If the answers suggest opioids are affecting your life, a licensed clinician can carry out a full assessment and recommend a plan.
